This kit and service is intended for the collection and shipping of baseline and concentrated human blood samples for research purposes. Samples should be stored at ambient room temperature.

Please note that all microtainers from a PAC kit need to be included in a single return shipment. PLYMOUTH MEDICAL reserves the right to forgo analysis of a sample due to materials not arriving intact due to improper packaging. 

Samples can only be shipped back to the lab Monday-Thursday.


PAC KIT


PAC DATA COLLECTION FORM & SAMPLE PREPARATION/PACKING

Access the online PAC Data Collection Form so you can enter the data as you collect your patient’s samples (Baseline Whole Blood (WB) and PRP sample). In the event that you are shipping multiple analyses in one PAC kit, complete the online PAC Data Collection Form as each PRP case is completed so no pertinent information is lost.

1. “Contact Details & Collection Details”

  • Complete required fields.

2. “Description of Baseline Whole Blood (WB) Sample”

  • Use the pre-labeled “Y” K2EDTA microtainers (eg: 1234Y) and collect the Baseline Whole Blood (WB) sample. Complete fields in the “Description of Baseline Whole Blood (WB) Sample” section of the PAC Data Collection Form. The microtainer number is your 5-Digit Sample ID (WB).

  • Baseline Whole Blood (WB) Collection: When recording volumes, use the total volumes collected from the patient - not what is placed in the K2EDTA Microtainer. You should not create a separate whole blood sample for analysis. Simply use 5-10 drops of the original sample for analysis. Place 0.25 mL to 0.5 mL (5-10 drops) of a well-mixed WHOLE BLOOD + ANTICOAGULANT sample into the microtainer using a syringe or transfer pipette immediately following blood draw. 

3. “Description of Preparation”

  • Complete required fields.

4. “Description of PRP”

  • Use the pre-labeled “Z” K2EDTA microtainers (eg: 1234Z) and collect PRP Sample. Complete fields in the “Description of PRP” section of the PAC Data Collection Form. The microtainer number is your 5-Digit Sample ID (PRP). If you observe persistent and visible strands of aggregated platelets, annotate this on the online Data Collection Form by checking the ‘YES’ box under ‘Platelet Aggregation / Strands Observed’.

  • PRP Collection: As with the Baseline WB sample, record the total volume of PRP produced - not what is contained in the K2EDTA microtainer. Place 0.25 mL to 0.5 (5-10 drops) mL of a well-mixed PRP sample into the microtainer using a syringe or transfer pipette.

5. “Description of Treatment”

  • Complete required fields and click on “SUBMIT DATA'“. Once submitted, a copy of the PAC DATA Collection Form will be sent to the email address on the form.

The final step is to properly pack your samples:

  • [1] Wrap the samples in the absorbent paper and place them back in the bag. Seal the bag by pressing the zip closed with your fingers. Do this a few times to ensure a tight seal. 

  • [2] Place the bag in the white box.

  • [3] Place the white box in the biohazard bag and seal Biohazard Bag by pressing the zip closed with your fingers. Do this a few times to ensure a tight seal. 

  • [4] Place biohazard bag in the pre-labeled FedEx UN3373 Bag.

  • Drop off box at any FedEx facility. 

    • The sample must be shipped by 5 PM to reach our Brooklyn lab by the next business day. Samples are typically stable for only 24 hours. Please note that samples can only be shipped back to the lab Monday-Thursday.

    • Due to the sample box being labeled as ‘UN3373’, the FedEx store may require that you place the box in the shipping container yourself.

If desired for internal purposes, document patient consent using this Patient Consent Form for blood sampling & testing for PRP characterization and quality control of protocols prior to submitting your samples.


ACCESS RESULTS
